Annual Mandated Student Information Forms

Students and Parents/Guardians,

Every student enrolled at Francisco Bravo Medical Magnet is required to complete mandated school forms as part of the enrollment process. The mandated school forms are to be returned individually for each student. Parents should ensure all student information is accurate and valid when completing the school forms. School forms can be completed and submitted online. 

  • Because this is an online process, signatures can be submitted by typing in the name of the parent, guardian, and/or student (depending on the form).  Your typed in name on the signature line and submission of the completed document constitutes  your acknowledgement, consent, and/or agreement to the form submitted.
  • To complete this process, we are asking that students work with family members to access and complete the forms and then upload them in digital format to a Google form accessible to students via their district SSO/email accounts. 
  • NOTE: ONLY students with LAUSD Email/SSO accounts will be able to access the UPLOAD links to the Google Form for submitting the forms for authentication and accounting purposes.
  • All forms are due by November 13, 2020.  Students whose forms are turned in by the deadline will be given priority for Student ID Card pick up.
  • Online forms, links and directions are provided below.
  • Students and families who are unable to access or complete the digital documents can request a printed packet of the documents. Please contact the assigned support person for the student's class (see left margin of this page) to request a packet.
  • Please contact the school at (323) 227-4400 for assistance and information or email the contacts listed in the left margin of this page.

To complete the process, find the box below that corresponds to your Class/Grade Level.

  • NOTE: Students in the Class of 2024/9th Grade have already submitted some of these forms and therefore have less forms below than other Classes/Grade Levels.
  • Access either the English (E-0x) or the Spanish (S-0x) versions of each one of the following documents by opening, downloading, completing (forms are fillable), and uploading to the link to the Google Upload Form linked below for your class/grade level.
  • For each form, it is recommended that you open the file, fill it out completely (you should preview the information you will need for each file), and then save the file to your computer or Google Drive.  If you are using a Chromebook, make sure to save the EDITED version once you fill out the form.  If you are using a Chromebook or the Chrome browser on a computer, you may want to use the KAMI extension app to complete the forms.
  • Save each completed form by titling the form by the letter/number code at the beginning of form title and then adding your last name:   Example: E-01-LastName
