Enrollment and Registration 2021-22

A message to all students who have been accepted to

Bravo for the 2021-2022 school year...

ENROLLMENT 2021-2022


Please complete forms #1-4 (click on each FORM NAME for each form) and gather documents #5-10 (please make copies of your original documents).


To complete your child's enrollment, please bring the 10 forms/documents to Bravo beginning August 2 from 8am-11am or 2pm-3pm. 


1. Enrollment Form

2. Emergency Information Form

3. Permanent Health History

4. Student Housing Questionnaire

5. Student's birth certificate

6. Utility bill (to verify residence)

*Other acceptable documents: property taxes, rental/lease agreement, current rental receipt with address of property on receipt, official government mail (CalWORKs Social Security, Medi-Cal), current pay stub, voter registration

7. Student's immunization card

8. Student's most recent report card or unofficial transcript

9. Parent's photo ID

10. Student IEP or Section 504 Plan and/or court orders (if applicable)



Students matriculating from an LAUSD school:

- may upload forms/documents onto the Parent Portal as an alternative to coming on campus. 

- If requested for a PIN, then please call your child's middle school to obtain it. 


Students matriculating from a non-LAUSD school:

- may upload forms on the Student Enrollment Portal at as an alternative to coming on campus.

- must also complete and submit the Meal Application form online:












Foreign Language Placement

Bravo’s Foreign Language Department offers Spanish and Mandarin to fulfill the graduation requirement. The department created the following Spanish assessment:

All students who wish to take Spanish as their Foreign Language must take the Spanish assessment for correct class placement. Please have your child click on the link to access the assessment, complete it, then submit their assessment. Students who wish to take Mandarin will not have to complete an assessment. Our Mandarin teacher determines and makes recommendations regarding class placement when she meets her students.




The District’s Transportation Unit will be sending notifications of home to school bus schedule in the mail in August before the opening of school for all students eligible for transportation . We anticipate that your child will receive transportation if your residence is outside the 5 mile radius of the school. You may check for transportation eligibility by clicking on this link to the radius map and entering your child’s home address in the search bar: Please contact the Transportation Office of our Area Bus Supervisor at (323) 223-6796 if you have not received any information in the mail a week prior to the start of school.    



Freshman Summer Bridge

Save the following dates: July 26th - August 6th for our Summer Bridge Program. The first week will be virtual and the second week will be in-person. The program’s details are dependent on Covid-19 related regulations. Please check our Bravo website   for upcoming information about registration for our Bravo Summer Bridge Program for incoming 9th grade students.




Sports Try-outs

Open an account at on August 1, 2021 or thereafter. Select CA, Francisco Bravo Medical Magnet, and your sport (you may select more than one sport). On this site, you will be filling out a variety of forms, most importantly the Sports Physical. This form must be stamped and signed by your doctor. Proof of medical insurance is also required and can be uploaded onto the site. All forms must be completed before you try out for the team. Try-out notifications will be posted on Schoology. 



The Escalante Program

The Jaime Escalante Program is only offering Geometry this coming summer. We apologize that there are no longer any spaces available in the program for our Freshman class and incoming students.  



Covid-19 Vaccination Program

Los Angeles Unified School District is expanding our Safe Steps to Safe Schools Initiative to include mobile vaccination to all Middle School and High School students ages 12-18 years old. Go to for the vaccine centers closest to you.
