ENROLLMENT 2022-2023

 

ENROLLMENT PROCEDURES

  • Pick up an enrollment packet at Bravo OR print out the enrollment forms (#1-5 below) that are accessible by clicking on their designated links. 

  • Complete all forms prior to your child’s enrollment date.

  • Gather and make copies of the additional required documents (#6-11 below). 

  • Ensure that all documents that are in another language are translated to English.

  • Bring completed copies of the following documents during your child’s enrollment date (see schedule below). 

  1. Enrollment Form

  2. Student Emergency Information Form *Include all health alerts in the designated area at the bottom of the form.

  3. Permanent Health History

  4. Student Housing Questionnaire (SHQ)

  5. Responsible Use Policy (RUP)

  6. Student's birth certificate

  7. Utility service bill (to verify residence) *Other acceptable documents: property taxes, rental/lease agreement, current rental receipt with address of property on receipt, official government mail (CalWORKs Social Security, Medi-Cal)

  8. Student's immunization card (Please include COVID vaccine card.)

  9. Student's most recent report card or unofficial transcript

  10. Parent's photo ID (please submit photo IDs for every parent that the student lives with)

  11. Student IEP or Section 504 Plan and/or court orders (if applicable)

ENROLLMENT DATES

See chart below for your assigned enrollment date and time. Your enrollment date is determined by your child’s last name which falls specifically within one of the last name ranges below. For example, if your child’s last name is CRUZ, then your enrollment date is May 12. Enrollment will take place from 4:00pm to 6:00pm each day. Please be sure to arrive no later than 5:45pm on your enrollment date.

World Language Placement

Bravo’s World Language Department offers Spanish and Mandarin to fulfill the graduation requirement. The department created the following Spanish assessment: https://forms.gle/6Et9E1qJUQd4BKeDA.

All students who wish to take Spanish as their Foreign Language must take the Spanish assessment for correct class placement. Please have your child click on the link to access the assessment, complete it, then submit their assessment as soon as possible before their enrollment date. Students who wish to take Mandarin will not have to complete an assessment. Our Mandarin teacher determines and makes recommendations regarding class placement when she meets her students.

 

 

Transportation

The District’s Transportation Unit will be sending notifications in the mail in August before the opening of school for all students eligible for transportation. We anticipate that your child will receive transportation if your residence is outside the 5 mile radius of the school. Please contact the Transportation Office of our Area Bus Supervisor at (323) 223-6796 if you have not received any information in the mail a week prior to the start of school.    

 

 

Freshman Summer Bridge

Bravo offers a Summer Bridge Program for our incoming 9th grade students. The program’s details are dependent on funding and Covid-19 related regulations. Sports Try-outs

Open an account at AthleticClearance.com. Select CA, Francisco Bravo Medical Magnet, and your sport (you may select more than one sport). On this site, you will be filling out a variety of forms, most importantly the Sports Physical. This form must be stamped and signed by your doctor. Proof of medical insurance is also required and can be uploaded onto the site. All forms must be completed before you try out for the team. Try-out notifications will be posted on Schoology. 

 

 

The Escalante Program

The Jaime Escalante Program is offering a Pre-Algebra class this coming summer for students who currently have a D or Fail in Math 8 in order to support them with additional preparation for Algebra 1 in high school.
